If you lost the census update form that was inside our Easter Sunday bulletin and subsequently available in our churches, you can download a copy from our Location and Contact page. The form, in PDF format, is accessible in the lower left column.

Print the form, fill it out, and drop it in the collection basket or mail it to the parish office address that is at the top of the form.

Those who prefer may fill out the form electronically and send it to the parish office via email. The parish general email address is also at the top of the form. If you don't have the software to fill out a PDF form, request a Word file from our webmaster.

Even if you think the office has your correct information, please fill out the form to update your parish census record so that we can verify it.
